Inpatient Rehabilitation and TCF
The staff members of the Transitional Care Facility (TCF) are committed to quality, compassionate care to help patients bridge the gap between an acute care setting and going home. Often medical-surgical patients no longer need treatment in an acute care hospital floor, but do require skilled rehabilitative care. The team of RNs, LPNs, and Certified Nursing Assistants provide general nursing care, lab and diagnostic services. A large part of the patient population on TCF are patients who have had total knee or hip replacement and who need more rehab before going home.
This 31-bed, state-certified skilled nursing unit is located on the 5th floor, just around the corner from the inpatient rehabilitation department. Our inpatient rehabilitation staff provides occupational, physical and speech therapies either in the patient's room or in the inpatient physical therapy area, depending on the patients needs. Patients are evaluated and accepted on admission based on Medicare Skilled Nursing Guidelines.
